Technical Assistance:
Use of pillows was discussed with the provider.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 1:15 PM and departed at 2:30 PM. Five children, totaling seventeen points, were in care with two caregivers. Nap time and afternoon activities were observed. No children's injuries were reported and it was reported that no medication has been administered to children in care. Two caregiver records and a sample size of five children's records were reviewed. An exit meeting was conducted with the provider prior to closure of the inspection.

Standard #: 22VAC40-111-140-B-3
Description: Based upon review of caregiver records, the provider has not ensured that the provider has o btained current cardiopulmonary resuscitation (CPR) certification. Evidence: The record provided for the provider indicates that the most recent CPR certification expired in March 2017. The online certification obtained does not meet Standards.

Plan of Correction: The provider responded with the following: First aid and CPR certification training will be scheduled for both the provider and the assistant.

Standard #: 22VAC40-111-140-B-4
Description: Based upon review of caregiver records, the provider has not ensured that the provider has obtained current first aid certification. Evidence: The record provided for the provider indicates that the most recent first aid certification expired in March 2017. The online certification obtained does not meet Standards.

Plan of Correction: The provider responded with the following: First aid and CPR certification training will be scheduled for both the provider and the assistant.

Description: Based upon review of caregiver records, the provider has not ensured that the assistant has obtained current cardiopulmonary resuscitation (CPR) certification. Evidence: The record provided for the provider indicates that the most recent CPR certification expired in March 2017. The online certification obtained does not meet Standards.

Description: Based upon review of caregiver records, the provider has not ensured that the assistant has obtained current first aid certification. Evidence: The record provided for the assistant #2 indicates that the most recent first aid certification expired in March 2017.
