A follow up was conducted for the initial inspection originally conducted on 09/21/2017. Please see today's issued violation notice for those cited areas of the standard.

Comments:
The following inspection took place between the hours of 2:30 pm through 4:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Ashburn, Virginia. At the start of this unannounced two month monitoring inspection there were a total of 8 children (7 points) with 1 caregiver. A total of 8 children?s files and 4 staff/household member?s files were reviewed. The children were engaged in the following: check-in, snack, bathroom break, hand washing, homework time, games, learning activities, pick up, and other organized activities. Standard #: 22VAC40-111-100-A
Description: Based on review, 1 of 8 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (12/04/2017), documentation of a physical examination was not available for review for Child #2. 2. The start date for Child #2 was listed as 08/24/2017. 3. Child #2 was in attendance on the date of inspection.

Plan of Correction: Documentation of a physical examination shall be obtained by the Parent of Child #2.

Standard #: 22VAC40-111-60-B
Description: Based on review, 3 of 8 children's files reviewed indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (12/04/2017), 1 of 2 emergency contact's addresses was not available for review: Child #2. There was 1 of 2 emergency contacts (with their information) completed for Child #5, and Child #6. 2. The name and policy number of the child's medical insurance was not available for Child #4. In addition, while the policy name was listed, the policy number for the following children's medical insurance was not available for review: Child #5, and Child #6.

Plan of Correction: All missing information shall be obtained and placed on file.
