Thank you for your time and assistance with the unannounced monitoring inspection conducted today, December 7, 2017. The on-site inspection was held from 10:25 a.m. to 12:55 p.m. On this date, there were four children in the care of the provider (11 points). There were a total of six children enrolled in the program. There were five children's records reviewed, along with the records for the provider and one household member. The provider and two children were interviewed. There were no medications being administered at the time of the visit, as is the policy of the provider. The exception to compliance was noted on the violation notice and the risk rating for the violation was noted on the supplemental page. Lunch was observed to meet USDA guidelines and consisted of meatballs, macaroni and cheese, corn, mandarin oranges, and milk. Some items reviewed/observed during the inspection included: required posted information, emergency supplies, first aid kit, documentation of evacuation and shelter-in-place drills, lunch, choice time, hand washing and bathroom procedures, infant feeding and care, diaper changing procedures, and nap. Standard #: 22VAC40-111-100-A Description: Based on review of five children's records and verification from the provider, the provider failed to obtain documentation of a physical examination for one child. Evidence: 1. The record for Child #1 did not contain documentation that a physical examination had been completed. First date of attendance for Child #1 was 11/01/2017. 2. The provider verified that the parent of Child #1 had not obtained a physical examination report. Plan of Correction: The provider contacted the parent of the child during the inspection and will obtain documentation of a physical examination as soon as possible.
