Technical Assistance:
Discussed fingerprint registry information. Also discussed 10 day requirement for CPS documentation verification.

Comments:
An unannounced inspection was conducted at 920 AM. There was one child present with the Provider for a total of 2 points. The Provider has completed over 20 hours in annual training, has MAT certification, and is current in CPR/First Aid certification. The assistant will need to complete 16 hours by the end of June, 2018. The entire home was checked, and the Provider's emergency supplies were complete and stored in one area. Standard #: 22VAC40-111-60-B
Description: Based on review of 3 children's files, it was determined that one file does not contain a completed emergency contact. Evidence: 1. Child #3 has two people lusted as emergency contacts but only one emergency contact includes the phone number.

Plan of Correction: I will request this when he comes.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on interview and record review, the Provider did not obtain a Child protective services central registry check within 30 days of a household member becoming 14 years old. Evidence: 1. Application lists household members and household member #2's birth date indicates the person is 14 years of age. 2. Provider stated the household member is 14 years of age and that she has not completed a background check for the household member.

Plan of Correction: I will print out the paper today and get it sent out by the end of the week.
