Discussion was held on the topics of current enrollment, current staffing, the new definition of a serious injury, and a review of changes to background checks with regards to expiration dates, new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years.
The following inspection took place between the hours of 1 pm through 3 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Sterling, Virginia. At the start of this unannounced monitoring inspection there were a total of 5 children (13 points) with 2 caregivers. A total of 5 children?s files and 3 staff/household member?s files were reviewed. The children were engaged in the following: lunch, freeplay, bathroom breaks, diaper changing, hand washing, nap time, and other organized activities. Standard #: 22VAC40-111-90-C Description: Based on review, 1 of 5 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (12/14/2017), the most current immunization documentation on file for Child #1 was dated - 03/24/2017. 2. Based on the information on file, Child #1 was under 2 years of age of the date of inspection. 3. Child #1 was in attendance on the date of inspection. Plan of Correction: An updated immunization page shall be obtained and placed on file for Child #1.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review, 1 of 3 staff/household member's files reviewed indicated that it has been more than three years since the date of the most recent criminal history record check (CRC) report. Evidence - 1. On the date of inspection (12/14/2017), a current CRC was not available for review for Member #1. 2. The most current CRC on file for Member #1 was dated - 11/07/2014. Plan of Correction: A request for an updated CRC was sent last month. After 30 days, a follow up phone call shall be placed and documented. If needed, a new request shall be sent. The Licensing Inspector (LI) shall be contacted when this documentation has been obtained.
Standard #: 22VAC40-191-40-D-5-a Description: Based on review, 1 of 3 staff/household member's files reviewed indicated that a Child Protective Services central registry check (CPS) was not conducted within 30 days of a person in the home becoming 14 years old. Evidence - 1. On the date of inspection (12/14/2017), based on documentation on file, Member #3 has turned 14 years old. 2. During the month of September would have marked 30 days prior to the date in which Member #3 would turn 14 years old. 3. During the date of inspection, a CPS check for Member #3 was not available for review. Plan of Correction: A request was sent last month to the central registry. After 30 days, a follow up phone call shall be placed and documented. If needed, a new request shall be sent. The Licensing Inspector (LI) shall be contacted when this documentation has been obtained.
