An unannounced monitoring inspection took place today from 11:15 - 12:40. There were six children in care for 14 points with the provider and assistant. I observed children napping and two children playing together (infants!) Record keeping was well-organized and the children's area was very inviting with a variety of age appropriate toys. If you have any questions, please contact me at firstname.lastname@example.org
Standard #: 22VAC40-111-960-E Description: Based on observation today, not all bottles were labelled with the child's name and date as required. Evidence: During the inspection, three bottles were observed to not have readable names on them. Plan of Correction: We will order different labels.
Standard #: 22VAC40-111-100-A Description: Based on a review of children's records today, one child in care did not have a physical exam within 30 days of attendance as required. Evidence: Child # 1, who began care 8/17, did not have documentation of a physical exam today. Plan of Correction: I texted his mother and she will bring it in.
Standard #: 22VAC40-111-60-B Description: Based on a review of a sample of children's records, emergency information was not reviewed and signed by the parent annually as required. Evidence: Child #2 did not have documentation of an annual review/update when it was due 10/17. Plan of Correction: I will talk to the parent today and have them review and sign before dismissal.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
