Comments:
An unannounced monitoring inspection was conducted today from approximately 9:40am-11:40am. There were 7 children in care (18 points) directly supervised by 2 caregivers. A sample of 4 children?s, 2 caregivers records and 2 household member's records were reviewed. The children were observed playing outside, watching television, eating yogurt and playing with toys. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-111-50-A
Description: Based on observation, written record of daily attendance was not kept. Evidence: The attendance for the following days were recorded: 12/11/2017; 12/12/2017; 12/13/2017; 12/14/2017; 12/15/2017; 12/18/2017; 12/19/2017; 12/20/2017

Plan of Correction: I will print more attendance sheets.

Standard #: 22VAC40-111-60-A
Description: Based on record review, children's records were incomplete. Evidence: 3 of 4 records reviewed were incomplete. 1) Child A was missing an annual update. 2) Child B was missing 1st day of attendance, medical insurance policy and an annual update. 3) Child C was missing father's work address and phone number, address of 1 emergency contact and an additional emergency contact.

Plan of Correction: Parents will complete the files.

Standard #: 22VAC40-111-320-B
Description: Based on observation, protective covers were not kept on electrical outlets. Evidence: Two outlets on a pillar near the cribs in the main play area did not have protective covers.

Plan of Correction: Corrected during inspection.

Standard #: 63.2-1722-B
Description: Based on record review and interview with provider, background checks for caregivers were not maintained. Evidence: A Criminal History Record Check and a Central Registry Check had not been initiated for a caregiver hired 07/11/2017.

Plan of Correction: I will submit these forms.
