- Technical Assistance:
Discussed a household member turning 18 in the upcoming months and the background check requirements for this family member. Discussed emergency contacts and updating children's information.
Today an unannounced Monitoring inspection was conducted in the afternoon between 1:00pm and 3:00pm. There were 6 children present with the provider and 1 assistant for a total of 17 points within the supervision guidelines. The children were observed napping. There is an ample amount of toys, books and age appropriate games and activities for the children. There is a separate napping area for the children who are sleeping in cribs upstairs. The rest of the children are sleeping downstairs. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Thank you for your cooperation. If you have any questions, please email me at email@example.com.
Standard #: 22VAC40-111-830-A Description: Based on record review and interview, the provider did not have any documented fire drills since the month of July. Evidence: The last documented fire drill was July 21, 2017. Plan of Correction: We have done them but I must have forgotten to write them down.
Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical exam for one child within the first 30 days of enrollment. Evidence: Child #1 first day of enrollment was 7/27/2015 and there is no documentation of a physical. Plan of Correction: I will ask the parents for a copy.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not contain all required information. Evidence: Child #2 and Child #4 files did not contain the families medical insurance policy number. Child #4 file did not contain a second emergency contact. All five children's records reviewed did not contain documentation of an annual review of emergency contact information. Plan of Correction: I will ask the parents for this information.
