Today an unannounced Monitoring inspection took place between 9:45am and 11:30am. There were 8 children present with the provider and 1 assistant for a total of 14 points within the supervision guidelines. The children were observed playing freely, singing, dancing and playing games. There is an ample amount of toys, games and supplies available to the children. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not contain all required information. Evidence: Child #3 file did not contain an address for the first emergency contact, a second emergency contact or a list of people authorized to pick up in case of an emergency. Child #5 file did not contain the families medical insurance policy number. Plan of Correction: I will ask the parents for this information this afternoon.
Standard #: 22VAC40-111-170-A Description: Based on record review and interview, the provider did not obtain a TB screening for one adult household member. Evidence: One adult household member (DOB: 10/30/1998) has not obtained a TB screening. The caregiver stated that she did not know that her household member needed to receive one. Plan of Correction: We will get it done.
Standard #: 22VAC40-111-180-A Description: Based on record review and interview, the provider did not obtain a Tuberculosis (TB) screening every two years for each caregiver. Evidence: Caregiver #1 TB screening is dated 7/18/2013. Caregiver #2 TB screening is dated is 5/26/2015. The provider convirmed that neither Caregiver has obtained a subsequent TB screening. Plan of Correction: We will get this done.
Standard #: 22VAC40-191-40-D-4-a Description: Based on record review and interview, the provider has not obtained a sworn disclosure statement or criminal history check for one adult household member. Evidence: Household member #3 (DOB: 10/30/1998) does not have any record of a sworn disclosure statement or criminal history check. The provider has confirmed that this adult household member has not completed these forms. Plan of Correction: We will complete these forms this weekend.
Standard #: VENDHOM-000-012-F Description: Based on record review and interview, one caregiver does not have current CPR and First Aid training. Evidence: Caregiver #2 does not have their CPR and First Aid certification. The provider has confirmed that this caregiver has not taken the course. Plan of Correction: I will look up with the next class will be.
