Technical Assistance:
Discussed with provider the changes to the background checks. Discussed the importance of obtaining a complete record before children start the program. Proved a fire drill form and asked that the provider use the English form.

Comments:
An unannounced monitoring inspection was conducted today between the hours of 11:00 am through 1:00. There were a total of 6 children (17 points) with 2 caregivers. A total of 3 children's files and 6 staff/household member's files were reviewed. The children were engaged in the following: toy play, games, music, and lunch. Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider has not reviewed and updated the emergency preparedness plan annually. Evidence: The emergency preparedness plan was last reviewed and updated 12/21/15.

Plan of Correction: The provider reviewed and documented the update this date.

Standard #: 22VAC40-111-830-B
Description: Based on record review, shelter in place procedures are not practiced a minimum of twice per year. Evidence: The emergency drills for 2017 were reviewed, a shelter in place drill was not practiced and documented for the year 2017.

Plan of Correction: I will start doing my shelter drills and write it on the form.

Standard #: 22VAC40-111-100-A
Description: Based on record review and provider interview, the provider has not obtained documentation of child's physical examination within 30 days after the first day of attendance. Evidence: Child # 3, first day of attendance 9/2017 does not have documentation of physical examination.

Plan of Correction: I will have the parents bring back the physical.

Standard #: 22VAC40-111-120-A
Description: Based on record review and provider interview, the provider has not maintained a record for each caregiver. Evidence: Caregiver # 4 started today, there is no record on file.

Plan of Correction: Caregiver #4 is a sub today, I will have her complete all the required forms.

Standard #: 22VAC40-111-60-A
Description: Based on record review, the provider has not maintained an up to date record for each child. Evidence: There is no record for child # 1 and #3. Provider stated that child #1 started 1/2/18 and child # 3 started 9/2017. Child # 2 record is incomplete, missing most of the information.

Plan of Correction: I will have the parents complete the forms today.

Standard #: 22VAC40-111-70-A
Description: Based on record review, the provider has not provided parents in writing the procedures of the family day home before the child's first day of attendance. Evidence: Child # 1, first day of attendance 1/2/18, child #2 first day of attendance 3/1/17 and child # 3, first day of attendance 9/2017 does not have documentation of the parent information.

Plan of Correction: I will have the parents complete the forms.

Standard #: 22VAC40-111-90-A
Description: Based on record review and provider interview, the provider has not obtained documentation of child's immunization before a child may attend the family day home. Evidence: Child # 3, first day of attendance 9/2017 does not have documentation of immunization record.

Plan of Correction: I will get the papers back from the parents.

Standard #: 22VAC40-111-170-A
Description: Based on record review and provider interview, the provider has not maintained at the time of hire a current Tuberculosis screening. Evidence: Caregiver # 4 started today, there is no documentation of a TB screening.

Plan of Correction: Caregiver # 4 was a sub today, I will have her get a TB before she works again.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider has not obtained a Tuberculosis screening every two years from the date of the last screening for every caregiver and household member. Evidence: The following TB screening have not been updated; caregiver #1, TB 11/10/15, caregiver#2 TB 6/23/15, caregiver #3 11/15/15, household member # 5 TB 11/16/15.

Plan of Correction: I will have everyone get their TB screenings.

Standard #: 22VAC40-191-60-B
Description: Based on record review and provider interview, the provider has not obtained a sworn statement for each caregiver prior to their first day of employment. Evidence: caregiver # 4 started today, there is no documentation of a sworn statement completed for the caregiver.

Plan of Correction: I will have the caregiver complete the form today.
