- Technical Assistance:
Discussed the shelter in place drill twice a year. The bulletin board with posted license and inspection reports had fallen down, please securely it to the wall. Discussed the importance on maintaining the attendance daily and the must be maintained for 2 years from the date of termination of services. Reminder again that each bottle must be labeled with child's name and dated. Reminded Provider to mail renewal application as soon as possible. Discussed the changes to the background checks.
An unannounced renewal inspection was conducted today between the hours of 9:00 am through 10:45. There were a total of 2 children (8 points) with 1 caregiver. A total of 2 children's files and 4 staff/household member's files were reviewed. The children were napping and bottle feeding during this inspection. Standard #: 22VAC40-111-960-E Description: Based on observation, bottles were not labeled with the child's name and date. Evidence: There were two children in attendance with bottles. The infant bottles were not labeled with the child's name or dated. Plan of Correction: I will label and date each bottle.
Standard #: 22VAC40-111-50-A Description: Based on record review, the family day home provider is not maintaining a written record of children in attendance each day. Evidence: The last documentation of daily attendance was dated 10/23/2017, attendance is not being maintained each day. Plan of Correction: I will keep a record of attendance.
Standard #: 22VAC40-111-60-B Description: Based on record review, the provider has not maintained a complete record for each child. Evidence: Two children records were reviewed, the following information was not maintained in one child's record. Child # 2 record did not have a street address for the second emergency contact. Child #2 record did not have a completed proof of identity information and the medical insurance policy number. Plan of Correction: I will contact the child's mom to get the information.
