Discussed proof of birth for children.
Today an unannounced Renewal inspection was conducted between 9:350am and 12:00pm. There were 12 children present with the provider and 1 assistant for a total of 32 points within the supervision guidelines. The children were observed sitting for circle time, working on their letters, coloring and having free play. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-800-B Description: Based on record review and interview, the provider has not reviewed the emergency preparedness plan annually as required. Evidence: There is no documentation of an annual update for the providers emergency plan since it's inception on 7/22/2010. The provider has confirmed that she did not know that she needed to review the plan. Plan of Correction: We will correct this today.
Standard #: 22VAC40-111-920 Description: Based on observation and interview, the provider did not have a menu posted. Evidence: When asked about the location of the food menu, the provider stated that she was currently working on it. Plan of Correction: A current menu was provided at the end of the inspection.
Standard #: 22VAC40-111-960-E Description: Based on observation, infant's bottles were not labeled with the child's full name and date. Evidence: There were two bottles in the refrigerator that were not labeled with a name or date. Plan of Correction: We have stickers to use. I will use them.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not contain all required information. Evidence: Child #1 file did not contain a second emergency contact or parents acknowledgement of emergency medical care. Child #2 file did not contact a phone number for their first emergency contact or parents acknowledgement of emergency medical care. Child #3 file did not contain the families medical insurance policy number. Child #4 file did not contain the street address for their second emergency contact. Plan of Correction: I will ask the parents for this information.
Standard #: 22VAC40-111-180-A Description: Based on record review, one household member did not obtain a Tuberculosis (TB) screening every two years as required. Evidence: Household member #3 most recent TB screening is dated 7/20/2015. Plan of Correction: I think she has already done this. I will ask for a copy.
Standard #: 22VAC40-111-680-B Description: Based on observation, children's hands were not washed with soap and water as required. Evidence: One child was observed having their diaper changed. The child's hands were not washed after their diaper change. Plan of Correction: We will make sure the children get their hands washed.
Standard #: 22VAC40-191-40-D-4-a Description: Based on record review, one household member has not completed a Sworn Disclosure Statement as required. Evidence: Household member #4 does not have a Sworn Disclosure Statement on record. Plan of Correction: I will get it today when they get back from work.
