An afternoon inspection was conducted today. There were 9 children present with the Provider and two caregivers for a total of 18 points. The children were waking up from nap, and preparing for snack. Hand washing and food prep was observed. Children's and staff records were reviewed. Please call with questions 703-479-4679
Standard #: 22VAC40-111-120-A Description: Based on record review of staff files, it was determined that the files are not complete. 1. Staff 1 does not have documentation to verify references are complete. It is missing the signatures of the person who conducted the references. 2. Staff 1 and 2 have Sworn Disclosure Statements that do not include whether one has lived outside of the commonwealth within the last 5 years. Plan of Correction: Both corrected.
Standard #: 22VAC40-111-90-A Description: Based on record review of children's records, the Provider has not obtained an immunization record for one child that began enrollment this week. Plan of Correction: We have requested this info.
Standard #: 22VAC40-111-690-E Description: Based on observation, caregiver did not use foot to operate lid on plastic lined trash can. Evidence: 1. Caregiver used foot to lift the lid up and then used hand to close the lid. Plan of Correction: Not available online. Contact Inspector for more information.
