Technical Assistance:
Discussed with provider the following: background checks, appropriate use of hand sanitizer, diapering procedures.

An unannounced monitoring inspection was conducted from 10:45 am through 1:15pm. There were 11 children (28 points) with 3 caregivers. A total of 5 children's files and 6 staff/household member's files were reviewed. The children were observed preparing for lunch time, eating lunch, getting diapers changed and using the restroom, story time as they transitioned to nap time. There is an ample supply of toys and maniupulatives for the children to use. The caregiver to children interaction was positive. The menu was posted and being followed. Today's lunch menu included fish sticks, pears, celery and milk. The daycare area was maintained in a clean and organized manner. Areas of non-compliance are identified in the violation notice. Description: Based on record review and staff interview, provider did not maintain documentation of the current rabies vaccination for house pets. Evidence: 1. Provider stated that the family got two dogs one month ago. 2. The provider stated that the rabies shots are up to date and the previous owner completed the vaccinations. 3. The provider has not collected the vaccination records from the vet. There were no rabies vaccinations on site.

Plan of Correction: I will pick up the vaccination record from the vet.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on record review and staff interview, provider did not obtain the central registry check for a household member within 30 days of the turning 14 years of age. Evidence: 1. Household member # 1's date of birth is 11/24/03. 2. A central registry check was was not submitted within 30 days of household member #1 turning 14 years of age. 3. The provider stated that she was not sure of the time frame during which the central registry check would have to be submitted.

Plan of Correction: I will submit the form by tomorrow.
