Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file. Based on the information on file, the Family Day Home (FDH) will be able to start their fingerprint checks on May 1, 2018. If you did not receive your Facility ID number, your Field Print positions numbers or you have not establish a point of contact, then please contact the Office of Background Investigations (OBI) at 1 (804) 726-7884.
The following inspection took place between the hours of 10 am through 1 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located near the town of Aldie, Virginia. At the start of this unannounced renewal inspection there were a total of 6 children (20 points) with 2 caregivers. A total of 7 children?s files and 3 staff/household member?s files were reviewed. The children were engaged in the following: toys play, books, block play, music, feeding, lunch, diaper changing, hand washing, nap time, and other organized activities. Standard #: 22VAC40-111-90-C Description: Based on review, 1 of 7 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (01/23/2018), the most current immunization page of file for Child #3 was dated 06/19/2017. 2. Based on the information on file, on the date of inspection, Child #3 was under the age of 2 years. Plan of Correction: The Parent of Child #3 was contacted on the date of inspection and informed the Provider that the most current shot record was completed in October 2017. The doctor's office will be mailing the FDH a copy of this documentation.
Standard #: 22VAC40-111-200-A Description: Based on review, 1 of 3 staff/household member's files reviewed indicated that the Provider did not have documentation of the orientation training on file by the end of the staff person's starting week after assuming their job responsibilities. Evidence - 1. On the date of inspection (01/23/2018), an orientation page was not available for review for Staff Member #2. 2. The Provider stated (during an interview conducted on the date of inspection) that the start date for Member #2 was January 9, 2018. Plan of Correction: An Orientation page shall be completed and placed on file for Staff Member #2.
