An unannounced monitoring inspection was conducted today from approximately 11am-12:50pm. There were 5 children (10 points) directly supervised by 2 caregivers. A sample of 3 children?s and 2 caregivers records were reviewed. The children were observed watching age appropriate television, and playing in the play area. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the assistant. Description: Based on record review, evacuation drills were not practiced monthly. Evidence: There was no evacuation drill practiced for July 2017.

Plan of Correction: Evacuation drills will be completed once monthly.

Standard #: 22VAC40-111-60-A
Description: Based on record review, children's records were incomplete. Evidence: 3 of 3 children'd records reviewed were incomplete. 1) Child A was missing the address of the second emergency contact, permission to travel away from facility. 2) Child B was missing fathers employer, work phone number and address, permission to travel away from facility. 3) Child C was missing all of the fathers information.

Plan of Correction: Parents will complete files.

Description: Based on record review, repeat tuberculosis screenings were not completed within the allotted two years. Evidence: 1) Caregiver 1's tuberculosis exam was last completed 05/13/2015 and expired 05/13/2017. 2) Caregiver 2's tuberculosis exam was last completed 05/13/2015 and expired 05/13/2017.

Plan of Correction: We will call for an appointment next Monday.

Description: Based on record review, repeat background checks were not completed before the required 3 and 5 year deadlines. Evidence: 1) Caregiver 1 Criminal History Record search was last completed 01/01/2013 and has expired in 01/01/2016 Central Registry Record search was last completed 12/18/2012 and has expired 12/18/2017 2) Caregiver 2 Criminal History Record search was last completed 12/06/2012 and has expired in 12/06/2015 Central Registry Record search was last completed 01/07/2013 and has expired 01/07/2018

Plan of Correction: Criminal History Record search for Caregiver 1 and the Central Registry Record search for Caregiver 2 have been submitted earlier this month. Other background checks will be complete tomorrow.

Description: Based on observation, the most recent license was not posted. Evidence: The most recent license was lying on the dining room table and not posted a the public entrance.
