Comments:
10 children were in care with the provider and two assistants, The total number of points was 28. The age range of the children was five months to ten years old. The children were observed during lunch, free play and rest. The house hold members records and five children's records were reviewed. Provider and one assistant completed 18 hours of training. Inspection time 10:30am -1:00pm.

Standard #: 22VAC40-111-280
Description: Based on observation not all poisonous materials were kept away form children. Evidence: The provider had spread ice melting material on the side walk used by the children, it had been tracked into the house where the children change their shoes. The bucket of ice melt was stored at the bottom of the stairs used by the children. The bucket had warning labels to keep away form children.

Plan of Correction: The bucket was removed and stored away from the children all the ice melt that was tracked in the house was cleaned up.
