Comments:
An unannounced monitoring inspection was conducted today from approximately 12:50pm-2:20pm. There were 7 children (18 points) directly supervised by 2 caregivers. A sample of 4 children?s and 2caregiver's records were reviewed. The children were observed during diapering procedures and napping. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the assistant. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records were incomplete. Evidence: 4 of 4 records reviewed were incomplete. 1) Child A was missing immunization and health records, and parents work phone number. 2) Child B was missing an annual update. File was last updated 6/1/2016. 3) Child C was missing the father's work phone number. 4) Child D was missing first date of attendance, signature and date from the parent, and health exam.

Plan of Correction: Parents will update files today.

Standard #: 22VAC40-111-140-B-3
Description: Based on observation, the first aid and cpr certifications were not current. Evidence: The first aid and cpr certification of Caregiver A and Caregiver B expired 1/20/2018

Plan of Correction: We will be retrained next week.

Standard #: 22VAC40-111-210-A
Description: Based on record review, an additional 16 hours of training was not maintained annually. Evidence: Caregiver A had not completed 16 hours of training annually. The latest training was completed on 1/9/2015.

Plan of Correction: I have another packet of trainings I am unable to find it. We will complete training annually.

Standard #: 22VAC40-111-510-C
Description: Based on observation, occupied cribs were not at least 12 inches from each other. Evidence: 3 occupied cribs were against the wall in a row touching one another.

Plan of Correction: I will pull them out next time.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the most recent violation notice was not posted. Evidence: The violation notice posted was conducted January/2017 and the last inspection was conducted July/2017.

Plan of Correction: Completed during inspection.
