- Technical Assistance:
The provider shall immediately review all previous emails regarding fingerprinting requirements. The provider shall also immediately contact OBI to provide point of contact information and then receive fingerprint codes from OBI. Children shall not be seated in their high chairs until the provider is ready to serve their meal.
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 5 children, totaling 17 points. Children's records, adult records, caregiver training records, children's nutrition, programming, emergency preparedness items, and the physical plant were inspected today. The provider reported to the Inspector that no children's medications were on site today. The children were observed finishing lunch, playing with toys, and taking their afternoon nap. Standard #: 22VAC40-111-920 Description: Based on a review of posted documents, it was determined that the posted menu does not include all required information. Evidence: The posted menu is not dated. Plan of Correction: I have remembered to date the menu, but I forgot this week.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1. Child record #1 does not include: each parent's work phone number; the last name for each emergency contact person; the parent's medical insurance policy number. 2. Child record #2 does not include: the mother's work phone number. 3. Child record #3 does not include: the last name for one emergency contact person. Plan of Correction: Each parent will be asked for the missing information.
Standard #: 22VAC40-111-200-A Description: Based on a review of caregiver training records, it was determined that the provider has not conducted orientation training as required. Evidence: The provider has not conducted orientation training for each of the assistant caregivers. Both of these caregivers began working in the role of assistant caregiver on 1/15/18. Plan of Correction: We will do the orientation training today.
Standard #: 22VAC40-111-320-B Description: Based on observations, it was determined that two electrical outlets were not covered. Evidence: In the basement bathroom, two electrical outlets on the wall next to the children's handwashing sink were not covered. Plan of Correction: I will cover them today.
Standard #: 22VAC40-111-340-C Description: Based on observations, it was determined that one of the children's bathrooms was not equipped as required. Evidence: There are no paper towels in the basement bathroom used by the children. Plan of Correction: I will put the paper towels in there today.
Standard #: 22VAC40-111-520-C Description: Based on observations, it was determined that the provider has been using one crib that has been recalled by the US Consumer Product Safety Commission. Evidence: The Inspector observed one infant sleeping in a crib that has a manufacture date of March 30, 2003. The manufacture date was marked on the label attached to the crib. Plan of Correction: I just bought that crib. I will get rid of it immediately.
Standard #: 22VAC40-111-520-E Description: Based on observations, it was determined that the provider has crib bumper pads in place. Evidence: Upon arrival, the Inspector observed two crib bumper pads in use in children's cribs. Plan of Correction: I will remove the bumper pads.
Standard #: 22VAC40-111-530-F Description: Based on observations, it was determined that soft bedding was used in an infant crib. Evidence: Upon arrival, one 6 month old infant was observed sleeping in a crib. There was a soft blanket, a soft pillow, and a stuffed animal toy inside the crib with the sleeping infant. Plan of Correction: The soft items were all removed during the inspection.
Standard #: 22VAC40-111-540-A Description: Based on observations, it was determined that two children were seated in a high chair without the safety strap securely fastened. Evidence: The Inspector observed two children seated in high chairs without the safety strap buckled. The Inspector asked the provider to fasten each safety buckle. Plan of Correction: The safety buckles were fastened during the inspection.
