- Technical Assistance:
An unannounced renewal inspection was conducted on Tuesday, January 30, 2018 from approximately 9:30AM to 11:00AM to determine the provider's compliance with licensing standards. In attendance were three non-resident children. The children were being supervised by one caregiver during the inspection. There was a total of six points based on the children?s ages. The children were observed playing with toys, coloring and building with blocks. The child care areas were filled with supplies and toys for the children. During the inspection, the licensing inspector reviewed the following: emergency supplies including first aid kit, battery operated weather radio, emergency plan, emergency drill logs, menu, caregivers? training hours and daily attendance. The provider stated medications are not being administered at this time. Transportation is not provided. During the inspection four children's, one caregiver?s and one household member?s records were reviewed. The risk profile tool will be used for the licensing recommendation by the licensing department. Standard #: 22VAC40-111-800-B Description: Based on documentation review, the provider did not document the review of the family day home's emergency preparedness plan.
During the inspection, the licensing inspector observed the family day home's emergency preparedness plan was documented as last being reviewed on 11/08/16.
Plan of Correction: I will review the plan today.
Standard #: 22VAC40-111-830-A Description: Based on review of documentation, the provider did not practice the required emergency evacuation procedures at least on a monthly basis. Evidence: During the inspection, the licensing inspector observed the emergency drill log documented a shelter-in place drill taking place in October 2017 but not an emergency evacuation drill. Plan of Correction: I will make sure I complete a fire drill every month, even the months I do a shelter in place drill.
Standard #: 22VAC40-111-100-A Description: Based on review of three children's records, one record did not contain documentation of a physical exam. Evidence: During the inspection, the licensing inspector observed the record for Child #3 (enrollment date 02/03/17) did not contain documentation of a physical examination by or under the direction of a physician prior to the child attending or within 30 days after the first day of attending the family day home. Plan of Correction: Provider has been requesting the physical exam from the parent. Parent was contacted during the inspection and parent will contact doctor and have the physical sent to the provider.
Standard #: 22VAC40-80-120-E-1 Description: Based on observation, the provider did not have the most recently issued license posted on the premises. Evidence: During the inspection, the licensing inspector asked the provider where the license was posted. The provider stated the license is kept in a binder. A modification inspection was conducted on 09/01/17 that modified the license reflecting the provider's current physical address. The provider is required to post the most recently issued license on the premises. Plan of Correction: I will buy a frame and post it on the wall.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation, the provider did not have the most recent inspection summary / violation notice posted on the premises.
During the inspection, the licensing inspector asked the provider where the inspection documents from the previous inspection were posted. The provider stated the inspections are kept in a binder. A monitoring inspection was conducted on 09/01/17. The provider is required to post the results from the most recent inspection on the premises.
