Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file. Based on the information on file, the Family Day Home (FDH) will be able to start their fingerprint checks on June 1, 2018, and August 1, 2018. Please, see the supplemental page issued today for more specific information. If you did not receive your Facility ID number, your Field Print code numbers or you have not establish a point of contact, then please contact the Office of Background Investigations (OBI) at 1 (804) 726-7884.

The following inspection took place between the hours of 11 am through 2 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in the town of Leesburg, Virginia. At the start of this unannounced renewal inspection there were a total of 5 children (15 points) with 2 caregivers. A total of 5 children?s files and 3 staff/household member?s files were reviewed. The children were engaged in the following: toy play, games, learning, infant sleeping, music, equipment play, lunch, diaper changing, hand washing, nap time, and other organized activities. Standard #: 22VAC40-111-100-A
Description: Based on review, 1 of 5 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (01/31/2018), a physical examination for Child #2 was not available for review. 2. The start date for Child #2 was listed as 11/13/2017.

Plan of Correction: The parent of Child #2 shall be contacted and a physical shall be obtained and placed on file.

Standard #: 22VAC40-111-120-A
Description: Based on review, 1 of 3 staff/household member's files reviewed indicated that the Provider did not maintain a complete record for each caregiver. Evidence - 1. On the date of inspection (01/31/2018), documentation that two or more references as to character and reputation as well as competency were checked before employment were not available for Member #3. 2. The start date for Member #3 was listed as 08/10/2017.

Plan of Correction: Two references shall be completed for Member #3 and placed on file.

Standard #: 22VAC40-111-60-B
Description: Based on review, 1 of 5 children's files reviewed indicated that not all of the children in care had all of their emergency contact information listed. Evidence - 1. On the date of inspection (01/31/2018), for Child #2, there was 1 of 2 emergency contacts listed. 2. The other space for the second emergency contact was left blank.

Plan of Correction: The parent of Child #2 shall be contacted and information for a second emergency contact shall be obtained.
