54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted with the provider and assistant beginning at 9:00 AM and ending at 10:50 AM. There were four children in care during the inspection. The children were ages 22 months, 3, 3, and 3 years old for a total of 9 points. The children were observed eating breakfast which consisted of waffles, sliced apples, and milk. The children were observed drawing and during free play. Handwashing procedures were observed. Four children?s records and three staff records were reviewed. The emergency preparedness plan and emergency drill logs were reviewed. There were no medications on site as the provider is not certified in Medication Administrative Training (MAT). The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent emergency escape drill was documented as January 10, 2018. Standard #: 22VAC40-111-60-B Description: Based on a review of documentation, the provider did not obtain documentation of the annual review of the child's emergency contact information as required. Evidence: Child A's record did not have annual parent signature acknowledging the review of the child's emergency contact information. Plan of Correction: The provider will have the parent review the emergency contact information and sign the form tonight during pick up.
Standard #: 22VAC40-111-180-A Description: Based on a review of documentation, not all the staff members had documentation of a tuberculosis screening every two years as required. Evidence: Staff A's most recent tuberculosis screening form was dated December 01, 2015. Plan of Correction: The provider will have the staff complete an updated tuberculosis screening.
