- Technical Assistance:
Discussed with provider the importance of proper hand washing, cribs and background checks.
An unannounced monitoring inspection was conducted today between 10:30 am to 1:30 pm. There were 8 children (28 points) in care of two caregivers. The children were observed playing with toys, being fed lunch and bottle fed, taking naps in the cribs. The provider has an up to date record of emergency drills. Standard #: 22VAC40-111-720-A Description: Based on observation, the provider had non-prescription medication that was labeled and expired. Evidence: 1. A non prescription medicine for Child #3 was not labeled with the child's name. 2. The non prescription medicine's expiration date was 9/2015. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-111-750-A Description: Based on observation, topical skin ointments were expired and not labeled. Evidence: 1. Diaper cream for Child # 3 expired in 4/2017. 2. Skin moisturizer for Child # 4 was not labeled with the child's name. Plan of Correction: I will make sure that the ointments are kept current with dates and names.
Standard #: 22VAC40-111-890 Description: Based on observation, food brought from home was not labeled. Evidence: Children's food brought from home was not labeled with the children's names. Plan of Correction: I will ask the parents to label the food.
Standard #: 22VAC40-111-960-E Description: Based on observation, infant bottles with milk were not dated. Evidence: 1. 2 of 2 bottles for Child # 1 were not dated. 2. One bottle for Child # 6 was not dated. Plan of Correction: I will ask the parents to date the bottles.
Standard #: 22VAC40-111-100-A Description: Based on record review, provider did not obtain documentation of a physical exam for children within 30 of attendance. Evidence: 1. A complete physical exam was not obtained by the provider for Child # 5. The child's enrollment date is 7/17/17. 2. A complete physical exam was not obtained by the provider for Child # 6. The child's enrollment date is 7/17/17. Plan of Correction: I will ask the parents to bring in the physical for both children.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not include the medical insurance policy number. Evidence: Child # 5 and Child # 6 were missing the medical insurance policy number. Plan of Correction: I will obtain the information from the parents.
Standard #: 22VAC40-111-200-C Description: Based on record review, there was no documentation of orientation on file for the assistant. Evidence: Staff # 2's record did not have documentation of orientation. Plan of Correction: I did complete the orientation when she was hired, but I will do it again and complete the paperwork .
Standard #: 22VAC40-111-320-A Description: Based on observation, small appliances not in use were not unplugged. Evidence: Two small appliances in the kitchen upstairs were not unplugged while not in use. Plan of Correction: I will make sure that all small appliances are unplugged while not in use.
Standard #: 22VAC40-111-510-C Description: Based on observation, occupied cribs were not 12 inches apart. Evidence: 4 infant occupied cribs were not spaced at 12 inches apart during nap time. Plan of Correction: I will make sure that the cribs are 12 inches apart when in use.
Standard #: 22VAC40-111-680-B Description: Based on observation, caregivers did not ensure that the children's hands were washed with liquid soap and warm running water. Evidence: 1. Before lunch started, children staff # instructed staff # 2 to wash children's hands. 2. Staff # 1 gave Staff # 2 a bowl containing only water and small pieces of paper towel. 3. Staff # 2 took the bowl to the first child to rinse hands with the water and then dried the child's hands with the paper towel. She then dumped the water in the sink, and refilled the bowl with water to take to the next child. 4. The appropriate method of hand washing was not followed by both staff members. Plan of Correction: We will use the appropriate method of hand washing.
