Technical Assistance:
Discussed changes to background check regulations regarding fingerprinting and a possible provider change of address in the future.

Comments:
An unannounced monitoring inspection was conducted today from 8:30am-10:00am. At the time of inspection, 9 children (27 points) were directly supervised by the provider and two assistants. The sample size consisted of 5 children records, 3 caregiver records and 1 household member record. Children were observed during reading time, engaged in group play, and infants were observed napping. The provider is preparing a nutritious lunch of rice, beans, sour cream, apples, bananas, and milk for lunch today. Diapering and handwashing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. Repeat violation was found and is identified in this report. Standard #: 22VAC40-111-100-B
Description: Based on review of five children's records, the family day home did not have documentation of a physical in each child's file prior to or within 30 days of first attendance. Evidence: Child #1 (start date: 6/2017) and Child #2 (start date: 8/2017) were missing documentation of a physical examination within 30 days of the first day of attendance.

Plan of Correction: Parents were emailed at the time of inspection.
