Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file.

The following inspection took place between the hours of 11:30 am through 4 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Chantilly, Virginia. At the start of this unannounced renewal inspection there were a total of 6 children (18 points) with 3 caregivers. A total of 8 children?s files and 6 staff/household member?s files were reviewed. The children were engaged in the following: toy play, games, diaper changing, lunch, nap time, snack, and other organized activities. If you should have any future questions, please call. Thank you. Standard #: 22VAC40-111-790
Description: Based on observation, not all of the required telephone numbers were posted. Evidence - 1. On the date of inspection (02/05/2018), the responsible person for emergency backup care as required in 22 VAC 40-111-800 A 3; name and phone number was not posted. 2. This name and phone number was observed on the Family Day Home's Emergency Preparedness & Response Plan (EP&RP).

Plan of Correction: The EP&RP shall be posted; displaying this number.

Standard #: 22VAC40-111-800-B
Description: Based on observation, the Provider did not review the Family Day Home's Emergency Preparedness & Response Plan (EP&RP) and document in writing each review and update. Evidence - 1. On the date of inspection (02/05/2018), the most current review for the Family Day Home's Emergency Preparedness & Response Plan (EP&RP) was dated - 05/09/2016. 2. The dates for the review for Member #2, and Member #3 was also not documented.

Plan of Correction: Corrected on the date of inspection.

Standard #: 22VAC40-111-90-C
Description: Based on review, 1 of 8 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (02/05/2018), the most current immunization documentation for Child #2 was dated - 06/12/2017. 2. Based on the information on file, on the date of inspection, Child #2 was under the age of two years. 3. Child #2 was on site receiving care on the date of inspection.

Plan of Correction: The Parent of Child #2 shall be contacted and an updated immunization page shall be obtained.

Standard #: 22VAC40-111-200-A
Description: Based on review, 2 of 6 staff/household member's files reviewed indicated that the Provider did not document the orientation training for each assistant by the end of their first week of assuming job responsibilities. Evidence - 1. On the date of inspection (02/05/2018), orientation documentation was not available for review for Member #2, and Member #3. 2. The start date for Member #2 was listed as - 01/08/2018. The start date for Member #3 was listed as - 12/01/2017. 3. Both Member #2 and Member #3 were observed on site providing care for children.

Plan of Correction: Documentation shall be completed and placed on file for both Member #2, and Member #3.

Standard #: 22VAC40-111-280
Description: Based on observation, cleaning supplies were observed in an area accessible to children. Evidence - 1. On the date of inspection (02/05/2018), a lower cabinet located in the children's bathroom was observed unlocked. 2. Cleaning supplies were observed contained within this unlocked cabinet. 3. On the date of inspection, there were 6 children in care. Three of these children were preschoolers.

Plan of Correction: The cabinet was locked.

Standard #: 22VAC40-111-320-B
Description: Based on observation, 1 unused electrical outlet was observed without a cover. Evidence - 1. On the date of inspection (02/05/2018), one outlet was observed without its cover. 2. This outlet was located in the main child care area, behind the highchairs.
