An unannounced renewal monitoring inspection was conducted from 10:40 am through 12:45pm. At the time of entrance there were a total of 11 children (34 points) with 3 caregivers. A total of 5 children's files and 5 staff/household member's files were reviewed. The children were engaged in the following: playing with toys, reading books with caregivers, bathroom breaks, hand washing, having lunch and preparing for nap. The lunch menu included rice, baked chicken and milk. The caregiver to children interction was observed to be positive. The family day home area was observed to be clean and organized. Emergency plans/logs and first aid kit was maintained in an up to date manner. A walk through of the house was conducted and the outside play areas was observed to be in good condition. Standard #: 22VAC40-111-750-A Description: Based on observation, diaper ointment for a child did not have written authorization. Evidence: Child # 1's record did not have the written authorization for a diaper ointment. Plan of Correction: The child is not using the cream anymore and was discarded during the inspection.
Standard #: 22VAC40-111-750-D Description: Based on observation, diaper ointment for a child was expired. Evidence: 1. Child # 1's diaper ointment expired in 9/17. 2. Staff # 1 stated that the ointment was only for a rash and not used after that. Plan of Correction: The cream was discarded during the inspection.
Standard #: 22VAC40-111-70-A Description: Based on record review, medication policy in children's records were not updated. Evidence: 1. Staff # 1's medication administration training (MAT) expired on 11/26/17. 2. 5 of 5 children's records were reviewed and included the medication policy stating the provider will administer prescription and non prescription medication. 3. Children's records were not updated when the MAT for Staff # 1 expired. Plan of Correction: I will either update the forms or get the MAT training renewed.
Standard #: 22VAC40-111-90-C Description: Based on record review, child's record did not have updated immunization records. Evidence: 1. Child # 1's date of birth was 5/3/16. 2. The date of the most recent immunization records was 11/2016. Updated immunization records were not obtained by the provider from the parents. Plan of Correction: I will ask the parents to provide updated shot records.
