Discussed the following with the Provider: - new legislation regarding background checks - differences between being permitted through the county and state regulation/requirements.
An announced initial inspection began briefly on January 9th at 11am. This became a consultation visit as the Provider was not prepared for the initial inspection. The Licensing Inspector returned to the FDH to complete the initial inspection on Feb 1st at 10:50am. On 2/1, the physical plant, 3 caregiver/household member files, 3 children's files, caregiver training, child nutrition, children's activities, emergency procedures, and supplies were inspected today. There were 3 children (plus the Provider's child), totaling 14 points, under the supervision of 3 caregivers. During the inspection, children were observed enjoying free play and eating lunch. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served chicken, mashed potatoes, beans, mixed vegetables, and milk. Standard #: 22VAC40-111-920 Description: Based on observations, the menu did not meet all requirements. Evidence 1. There are two posted menus. Neither of which is dated. Plan of Correction: The Provider will date the menus in order to know which one is accurate.
Standard #: 22VAC40-111-100-A Description: Based on file review, 2 out of 3 children do not have documentation of a physical examination. Evidence 1. Children #1 and #2 do not have physical examinations on file. Plan of Correction: The Provider will ask the parents for this.
Standard #: 22VAC40-111-50-A Description: Based on interview, the Provider does not have a written record of children in attedance each day. Evidence 1. There is no attendance record. Plan of Correction: The Provider will use the sample form from the dss website to record this daily.
Standard #: 22VAC40-111-150-D-1 Description: Based on interview, 1 out of 3 caregivers is not currently certified in CPR. Evidence 1. Caregiver #3 is not certified in CPR. Plan of Correction: This caregiver is looking into finding a class to get certified.

Standard #: 22VAC40-111-280 Description: Based on observations, there was a disinfectant which was not stored in an area inaccessible to children in care. Evidence 1. In the bathroom, there was a bottle of Glade Air freshener on the back of the toilet. Plan of Correction: This will be moved.
