An unannounced monitoring inspection was conducted on February 12 2018 between 12:00pm and 1:30pm. There were 4 children (12 points) in the direct care of a provider and an assistant. Children were observed being fed lunch, being diapered, washing hands, and being placed down for nap. The day care space is clean and organized. Standard #: 22VAC40-111-90-C Description: Based on record review, 2 of 4 children's records reviewed did not contain documentation of additional immunizations for a child every 6 months for children under the age of two years. Evidence: Child #2, born 11/7/16, last documented immunization is dated 3/9/2017. Child #3, born 5/10/2016, last documented immunization is dated 3/21/2017. Plan of Correction: I will ask the parents for the paperwork.
