- Technical Assistance:
An unannounced monitoring inspection was conducted today from 10:00am-12:15pm. There were 12 children (30 points) directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, learning letters and numbers, building with blocks, reading books, working their fine motor skills. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-830-C Description: Based on review of emergency response drills records, the provider did not have documentation of an emergency escape drill for the month of January. Evidence: The drill log for the current year did not have documentation that a drill was conducted for the month of January 2018. Plan of Correction: I will conduct two drills this month.
Standard #: 22VAC40-111-920 Description: Based on observation and caregiver interview, the menu for the current one-week was not posted or given to parents. Evidence: The family day home serves snacks and meals, however a current weekly menu was not posted. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-111-60-A Description: Based on record review, 1 of 5 children's records does not contain an annual update of emergency contact information. Evidence: Child#2's record was last updated on 1/23/2017. Plan of Correction: I will ask the parents to review and sign the form.
Standard #: 22VAC40-111-60-B Description: Based on record review, 1 of 5 children's records did not contain all required information. Evidence: Child#5's record is missing the address and the phone number for the place of employment listed for the child's father, and the address for 1 of 2 emergency contacts. Plan of Correction: I will request the information from the child's parents.
Standard #: 22VAC40-111-90-C Description: Based on record review, the provider did not obtain additional immunizations once every six months for a child under the age of two years. Evidence: Child#1 (DOB:11/25/2015) has an immunization record on file that was last updated on 8/26/2016. Plan of Correction: I will ask the parents to update the immunization record.
