Technical Assistance:
New Sworn Affirmation forms were provided to the Vendor during today's inspection. In addition, contact Information for OBI was given to the vendor. Please call and give them your contact information and request a CODE for scheduling the fingerprint background checks to be conducted. Be prepared with an EMAIL ADDRESS for them to send the code to. In addition, you will need to schedule the fingerprinting on line one the CODE is received.

A Subsidy Health and Safety inspection was conducted on February 14, 2018 from 1:15 p.m. until 3:00 p.m. to review supplemental health and safety requirements. At the time of entrance, 6 children, ages 2 months to 5 years old, for a total of 18 points were in care with the vendor and one assistant. The records sample size consisted of 4 children?s records and 2 vendor / assistant/ household member records. Children were observed resting quietly on mats during rest period. Infants were awake and playing in infant activity seats, being held, cuddled and talked to. Medication administration was not observed. The vendor stated that there are no children being administered medication at this time, no children are routinely transported in a vehicle while in care and no accidents or injuries requiring first aid or medical care have been incurred by children in care since the last inspection. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found.
