Technical Assistance:
New Sworn Affirmation forms were provided to the Vendor during today's inspection. In addition, contact Information for OBI was given to the provider. Please call and give them your contact information and request a CODE for scheduling the fingerprint background checks to be conducted. Be prepared with an EMAIL ADDRESS for them to send the code to. In addition, you will need to schedule the fingerprinting on line one the CODE is received.

Comments:
An unannounced Monitoring Inspection was conducted on February 14, 2018 from 1:15 p.m. until 3:00 p.m. At the time of entrance, 6 children, ages 2 months to 5 years old, for a total of 18 points were in care with the provider and one assistant. The records sample size consisted of 4 children?s records and 2 provider / assistant/ household member records. Children were observed resting quietly on mats during rest period. Infants were awake and playing in infant activity seats, being held, cuddled and talked to. Medication administration was not observed. The provider stated that there are no children being administered medication at this time, no children are routinely transported in a vehicle while in care and no accidents or injuries requiring first aid or medical care have been incurred since the last inspection.
