Comments:
An unannounced monitoring inspection was conducted on January 24 2018 between 10:45am and 12:45pm. There were 12 children in the direct care of 3 staff members. Children were observed being fed lunch, having their hands washed, being diapered, watching an age appropriate television program. Staff were observed to be consistently engaged with the children. There is plenty of toys, books, and activities for the children to participate in. The family day home is clean, organized, and has plenty of space for the children to play. Description: Based on record review, 1 of 3 staffing records did not contain all of the required information. Evidence: Staff #2 record does not have documentation of an emergency contact, the contact's telephone number, or the contact's address.

Standard #: 63.2-1720.1-B-3
Description: Based on record review, 1 of 3 staffing records reviewed did not have documentation that a central registry search was requested for a staff member who lives or has lived outside the state of Virginia in the preceding 5 years. Evidence: Staff #2 currently lives in Washington D.C., but a request was not made for that locality.
