Comments:
An unannounced monitoring visit was conducted today between the hours of 9:06am am through 11:10am with the provider on site. There were 6 children (15 points) with the provider. A sample of 3 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play and infant care. The home is clean and organized. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child C started with the provider on 4/27/2017 and did not have documentation of a physical examination prior to the child's attendance or within 30 days after the first day of attendance

Plan of Correction: The parent of child C was present during the inspection and the provider requested that she provide a copy of the child's physical. The parent stated she would get a copy of it.

Standard #: 22VAC40-111-60-B
Description: Based on review, two of three children's records were not complete. Evidence: 1. Child B was missing both parent's work phone number. 2. Child C was missing name and address of previous child care and results of health exam.

Plan of Correction: The parent of Child B filled out the information while the inspector was at the family day home. The parent of child C filled out the name of previous child care while the inspector was at the family day home. The provider requested that the parent of child C provide a copy of the child's physical. The parent stated she would get a copy of it.

Standard #: 22VAC40-111-70-A
Description: Based on review, before the child's first day of attendance, parents were not provided all required information. Evidence: 1. Child B (start date 09/20/2017) and Child C (start date 4/27/2017) did not have documentation of the parent's signature on the liability insurance form prior to the child's first day of attendance.

Plan of Correction: Both parents signed the liability insurance form while the inspector was at the caregivers home.

Standard #: 22VAC40-111-80-A
Description: Based on review, the provider did not obtain from the parent name and location of previous day care programs and schools the child has attended within seven business days of the child's first day of attendance at the family day home. Evidence: 1. Child C started with the provider on 4/27/2017 and did not have the name and location of previous day care programs and schools the child has attended within seven business days of the child's first day of attendance at the family day home.

Plan of Correction: The parent of child C filled out the name of previous child care while the inspector was at the family day home.
