Technical Assistance:
An exit interview was conducted with the provider to discuss the results of today's inspection. Consultation was given regarding annual training hours, transportation requirements, and updated background check regulations.

An announced initial inspection was conducted today, February 13, 2018 to determine the provider's compliance with family day home regulations for possible licensure. The inspection was initiated at 11:15am and concluded at approximately 1:20pm. In attendance were two children totaling four points being supervised by the provider and assistant. Children were observed resting on mats then later prepared for lunch. Today's lunch consisted of plant based burgers, mixed vegetables, broccoli, wheat bread, and milk. All rooms of the home were inspected with no hazards found. Children's books, toys, equipment, and furnishings were all inspected and appeared to be age appropriate in operable condition. The family day home had the required postings displayed on the wall upon entering the children's room. The first aid kit, fire extinguisher, and emergency supplies were inspected. One child's record and all provider records were inspected with the majority of the required information found. The following was also inspected: evacuation log, emergency preparedness plan, parent agreements, and other pertinent information. Description: Based on the inspection of provider records, the required information was not listed. During the inspection of two provider records, it was discovered the assistant did not have two or more references checked prior to the start of employment.

Plan of Correction: Per provider: I will have the references checked by the end of the week and place the documentation in the record.

Standard #: 22VAC40-111-200-C
Description: Based on the inspection of provider records, the required information was not available to review. Evidence: During the inspection of provider records, it was discovered the assistant did not have any documentation of orientation.

Plan of Correction: Per provider: I will go over the documentation of assistant orientation form with my assistant and we both will sign the form.
