Technical Assistance:
Provided number to Office of Background Investigations to ensure receipt of staff and household codes for fingerprint searches. You are reminded that all fingerprinting for staff and household members are required by September 2018. You are reminded that Central Registry and Sworn Statements and completed fingerprint results are now effective for 5 years and not 3 years. A reminder that documentation of new staff orientation training is required within the first 7 days of hire.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 9 children (27 pts) with Provider and 1 Assistant. Children were observed singing and dancing to music, coloring at table tops and free playing. Three infants sleeping in the crib room. Observation of the crib room and programming area was made. Children enjoyed playing in the mesh school bus, taking turns crawling through the bus while singing "The Wheels on the Bus". Younger children were observed being held by staff. No medications on site, no food allergies identified. A total of 6 children's records, all staff and household member records were reviewed. This inspection began at 9:30am and ended at 11:20am. Standard #: 22VAC40-111-90-A
Description: Based on child record review, documentation of immunizations were not obtained for one child. Evidence: Child # 4 began care on 8/14/17. No documentation that this child has been properly immunized was maintained.

Plan of Correction: I will request the current immunization record from the parent today.

Standard #: 22VAC40-111-90-C
Description: Based on child record review, documentation of additional immunizations was not obtained. Evidence: Child # 6 is 14 months old. The last documentation of immunizations was dated June 2017. Documentation of updated immunizations is required every 6 months for children under the age of 2 years.

Plan of Correction: I will request updated immunizations for all children in care.

Standard #: 22VAC40-111-280
Description: Based on observation of the home, one potentially poisonous substance was found accessible to children in care. Evidence: A bleach spray cleaner was found under the cabinet sink in the downstairs bathroom unlocked and accessible to children.

Plan of Correction: Provider removed this cleaning product immediately
