Technical Assistance:
Reviewed the new fingerprinting process. Contact OBI (office of background investigation) at 804-726-7884 to obtain your field print code to begin fingerprinting. All background checks are good for 5 years from the date on the last form.

Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:30 am through 12:30. There were a total of 5 children (10 points) with 2 caregivers. A total of 4 children's files and 6 staff/household member's files were reviewed. The children were engaged in the following: toy play, books, and coloring. Violations:
Standard #: 22VAC40-111-60-A
Description: Based on record review and provider interview, the provider has not maintained a complete record for each child. Evidence: The provider has not obtained a record for child #1 who is a family member but not living in the house.

Plan of Correction: I will have the child's parents complete the papers for child #1.

Standard #: 22VAC40-111-60-B
Description: Based on record review, a child's record does not have all the required information on file. Evidence: Child #2 record does not have 2 emergency contact name, address and phone number.

Plan of Correction: I will get information from the parents.

Standard #: 22VAC40-111-210-A
Description: Based on record review and provider interview, the provider has not completed 16 hours of annual training. Evidence: There is no documentation of annual training for the provider, the provider stated that she has not taken any training.

Plan of Correction: I will start taking some training.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diaper changing pad is not non absorbent. Evidence: The diaper changing pad was found with several tears making it a nonabsorbent surface.

Plan of Correction: I will buy a new one.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on record review, the provider has not obtained background checks for each household member. Evidence: There is no criminal history record for household # 3. There is no central registry search for household #2.

Plan of Correction: I will re send the central registry form today. I will have her get fingerprinted by this weekend.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the provider has not obtained an original central registry finding within 30 days of employment for each caregiver. Evidence: Assistant #1 does not have a current central registry finding within 30 days of employment. Date of hire 9/23/2016.

Plan of Correction: I will re send the central registry form.
