Comments:
Today an unannounced monitoring inspection was conducted with the Provider. This visit took place in the morning between 9:30-11:30 there were 6 children with the Provider for a total of 16 points within the supervision guidelines. The children were observed having morning free play, playing with puzzles, morning snack, nap, and in transition for lunch. Lunch consisted of hotdogs,pranta, mashed potatoes, milk and oranges. Positive interactions were observed during this inspection. A complete inspection of caregiver records, children records, fire drill log, attendance, menu and emergency procedures. Standard #: 22VAC40-111-960-E
Description: Based on observation, infant bottle was found without name and date. Evidence: Two infants were in care and the bottles did not include the name and date.

Plan of Correction: Provider labeled the bottles during this inspection.

Standard #: 22VAC40-111-200-C
Description: Based on observation, no documentation of orientation training was signed and dated. Evidence: Caregiver #1's record, did not include documentation of orientation training.

Plan of Correction: Provider stated that she will complete it today.

Standard #: 22VAC40-111-680-B
Description: Based on observation, children hands were not washed with soap and water before feeding them. Evidence: Children were observed being served crackers however theirs hands were not washed first.

Plan of Correction: Caregiver washed hands during this inspection.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on review of one staff records, the provider did not have documentation of a central registry check report within 30 days of employment. Evidence: Caregiver #1's record did not include CPS CHECK.

Plan of Correction: Provider stated that she completed it however she is awaiting the results.
