An unannounced monitoring inspection was conducted today, March 13, 2018 to determine the providers' compliance with the Licensing standards. The inspector arrived at approximately 2:00 pm and concluded the inspection at 5:00 pm. Upon the inspectors arrival, three family day home children were in care with two of the provider's own children. children were just getting up from nap. Children had bathroom breaks and diapering was conducted. Shortly after two additional family day home children arrived from school with two of the provider's school age children. A total of five family day home children and four residential children were in care with the one provider for a value of 15 points. Children colored and watched a children's video. Later children had a snack of crackers and sliced pear wedges after washing their hands. The provider and children played a group game of musical chairs. The provider incorporated basic math in the game reviewing the number of chairs after they removed a chair each time.All areas of the home used for child care were observed. The family day home has age and stage appropriate materials and supplies. Children do not use the rear deck at any time. The first aid kit and non medical emergency supplies were inspected and found complete. The family day home had the daily schedule, menu, License, last inspection notice and emergency numbers posted. A smoke detectors and two fire extinguishers were observed. The inspector interacted with the provider and children in care. One caregiver record and five children's records were reviewed during today's inspection (see violation notice). Medication is not being administered. The provider has current CPR and first aid training. The last documented emergency evacuation drill was held on 02/22/2018. The two required shelter-in-place drills for 2018 were completed on 01/09/2018 and 02/22/2018. Per the provider: no pets , no transportation. Violations cited during the previous inspection were checked for corrections. There was one repeat violation cited during today's inspection. Standard #: 22VAC40-111-100-A
Description: Based on review of children's records, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child #4, with an enrollment date of 01/30/2018, did not have documentation of a physical on file.

Plan of Correction: Per the Provider: I will ask the grandparent to get me a copy of the physical by the end of the week.

Standard #: 22VAC40-111-70-B
Description: Based on review of children's records, the provider did not obtain the parent's written acknowledgement of the receipt of the "Information for Parents". Evidence: The record of child #3 did not have documentation of the parents written acknowledgement of the "Information for Parents".

Plan of Correction: Per the Provider: I will have the parent sign the acknowledgement for the parent Information.
