Discussed the need to maintain certifications in an easy accessible location.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 12 children (28pts) with 2 assistants present. Provider currently on vacation. Observation of the entire programming area to include the crib room was made. When the inspector arrived the older children were completing table top activities such as tracing baby chicks and coloring them. A nice transition to free play on the carpet was made where children either played independently, with staff or in groups of children. Ample amounts of supplies, toys and books available. The children participated in clean up of the free play time and staff provided circle time which involved singing preschool songs. Children lined up behind staff afterwards and proceeded to hand washing prior to lunch. Lunch today was homemade meatloaf, rice and green beans. Ample amounts available. Children were provided adequate time to complete their lunches. Positive interactions were observed between staff and the children in care. Children familiar with the routine and schedule of the day. A total of 6 children's records and all staff files was made. No medications on site. Thank you for your cooperation today. This inspection began at 9:35am and ended at 12pm. Standard #: 22VAC40-111-60-B
Description: Based on child record review and phone interview with provider, some records were found incomplete. Evidence: The provider stated that proof of identity was verified with each parent, however documentation of the verification was not found for the following records: Child # 1,3,4 and 6.

Plan of Correction: Provider will request the birth certificates again from those parents and maintain documentation of proof of identity.

Standard #: 22VAC40-111-180-A
Description: Based on staff record review, a subsequent TB screening was not completed. Evidence: Staff # 2's last documented TB screening is dated 11/2015. A TB screening is required every 2 years.

Plan of Correction: This staff will have a TB screening this month.

Standard #: 22VAC40-111-230-A
Description: Based on staff record review, written documentation was not maintained. Evidence: Staff #3 has current CPR/First Aid, however no documentation of training was maintained in the designated record.

Plan of Correction: I will re locate the certification cards for this staff back in their staff record.

Standard #: 22VAC40-111-280
Description: Based on observation of the physical plant, potentially poisonous substances were found accessible to children. Evidence: Cleaning agents were found unlocked in the hall closet and under the cabinet under the sink in the bathroom located on the lower level of the home. Both of these areas are accessible to the children in care.

Plan of Correction: We removed all of these cleaning agents during the inspection. We will place a door knob lock on the hallway closet and a latch lock on the cabinet in the bathroom.
