Technical Assistance:
You are reminded of the Code changes with regards to background searches that the Sworn Statement, Central Registry and once the fingerprint searches are complete, all three background checks are effective for 5 years and no longer 3 years. As discussed please contact Office of Background Investigations at: (804) 726-7884 to request provider and household member codes to proceed with the fingerprint process. All fingerprint searches must be completed for all staff and household members by September 2018. You are reminded that no new hired staff are permitted on site until the fingerprint search results has been received.

An unannounced monitoring inspection was completed this date. A total of 6 children with Provider was present. The total number of points in care was 19 points, which placed the provider out of compliance with the maximum 16 points allowed. Walk thru of the programming area, observation of the two cribs in use and observation of diapering procedures was made. Two new children enrolled since the last inspection. A review of 6 children's records and Provider record was made. All records were found complete. Children were observed transitioning to nap time. Adequate spacing between cots was made. Provider observed holding one child in care and bottle feeding appropriately. Previous violations were reviewed from the November licensing inspection and corrective actions were verified. Description: Based on observation of the children in care, the provider has exceeded the 16 points allowed. Evidence: During the inspection there were a total of 6 children in care for a total of 19 points with the Provider.

Plan of Correction: I will ensure that I stay within the 16 points per caregiver. I will obtain additional staff.
