An unannounced renewal inspection was completed on 13/20/2018. There were six children in care with the licensee. The point total for the six children was 15. Five children's records were reviewed, the provider's record was reviewed, assistant caregiver's and the household member's record was reviewed. No medications at the family day home. The inspector arrived at the provider's home at 9:20 am and departed at 1:00 pm. The children were observed during free time, during a music and movement activity, story time, during lunch, and during nap. Discussion with licensee about: new sworn statement, finger prints and appointment scheduled for March 26, and about using the word reunite or reunification of the children to their parent or guardian for the emergency plan (fire, shelter in place and lockdown).
Standard #: 22VAC40-111-140-C Description: Based on review of the substitute provider log, the licensee failed to ensure the use of a substitute provider to be used no more that 240 hours per calendar year. Evidence: 1. The substitute provider log stated that there were 271 hours for the calendar year of 2017. Plan of Correction: The assistant will be work more than 240 hours per calendar year and hours will be monitored.
Standard #: 22VAC40-111-180-A Description: Based on review of records, the licensee failed to ensure that all members had a repeat TB screening every two years. Evidence: 1. Household members most recent TB screening was completed 2/23/2016. Plan of Correction: The household member will go and get the TB screening ASAP. All dates will be monitored.
Standard #: 22VAC40-111-280 Description: Based on observation the licensee failed to ensure that all poisonous substances were not stored with food. Evidence: 1. Observed on top of the refrigerator a spray can of of a deodorizer and a spray can of disinfectant stored with several items of food. Plan of Correction: The chemicals were moved during the inspection and will be stored in a place not around food.
