Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees and household members must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file.

Comments:
The following inspection took place between the hours of 9:30 am through 12:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 8 children (23 points) with 2 caregivers. A total of 9 children?s files, 2 staff files, and 4 household member?s files were reviewed. The children were engaged in the following: toy play, art, music, learning, diapering, bathroom break, hand washing, lunch, nap time, and other organized activities. Standard #: 22VAC40-111-70-A
Description: Based on review, 2 of 9 children's files reviewed indicated that not all of the documentation/information was contained within each child's record. Evidence - 1. On the date of inspection (03/22/2018), the Policy for the Administration of Medications form for the Family Day Home (FDH) was not available for review for Child #3, and Child #9. 2. This form was missing for these 2 newer children.

Plan of Correction: This form shall be given to the parents of all applicable children, completed, and placed on file.

Standard #: 22VAC40-111-90-C
Description: Based on review, 1 of 9 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (03/22/2018), a current immunization page was not available for Child #8. 2. The most current immunization page on file at the FDH was dated - 07/26/2017. 3. On the date of inspection, Child #8 was under the age of two years.

Plan of Correction: A current immunization page shall be obtained from the parent of and for Child #8. This documentation shall be placed on file.
