Discussed placement of violation notice.
Today an unannounced Monitoring inspection took place between 2:20pm and 3:40pm. There were 11 children present with the provider and 2 assistant for a total of 28points within the supervision guidelines. The children were observed waking up from their naps, washing their hands and playing outside. Today the children are dying Easter eggs. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-960-E Description: Based on observation, bottles were not labeled with children's full name and date. Evidence: There were two bottles observed on the counter downstairs that did not contain a date. Plan of Correction: I will start doing it for the parents.
Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain a physical exam for one child within the child's first 30 days of attendance. Evidence: Child #3 first date of attendance was 1/22/2018 and there is no documentation of a physical exam on record. Plan of Correction: I will tell the parents tonight that the doctor needs to complete the form.
