Technical Assistance:
The LI provided the phone number of Office of Background Investigations (OBI) and explained the procedures to follow in order to obtain a facility code and to schedule an appointment for a national fingerprint background check. The LI assisted the Provider on her computer to search the DSS public web site and to print an emergency preparedness form.

An unannounced monitoring inspection was conducted in the Family Day Home on March 20, 2018. There were 3 children in care. The children's ages ranged from 5 months to 4 years. Based on the ages of the children, the point total was nine. A sample of 5 records was reviewed. Information was reviewed for the caregiver to verify compliance from the last inspection report. Documentation of the updated TB test was reviewed. The children were observed after nap time. The children were served an afternoon snack. A diapering procedure was observed. Findings of the inspection were reviewed at exit. The inspection was conducted from approximately 2:40pm to 5:50pm. If you have any questions, please call (540)309-2310. Thank you.

Description: Based on a sample review of children's records, the Provider failed to ensure emergency contact information was reviewed with the parent on an annual basis and the parent's signed acknowledgment of the review documented in the record. Evidence: The parent's signed acknowledgment of an annual review of the emergency contact information was not documented in the record of child 2.

Plan of Correction: The Provider will review the child's record with their parent to ensure emergency contact information is current. The parent will date and sign the form to verify all information is correct and current.

Description: Based on documentation reviewed for the emergency plan, the Provider failed to document an annual review. Evidence: The Provider had not documented an annual review of the emergency plan. The Provider reported that the plan is to be revised to reflect the current procedures that are followed, the children will not be taken to the basement of the home.

Plan of Correction: The Provider will revise the current emergency plan. Each parent will be given notice of the revised written plan. Documentation will be retained in each record.

Standard #: 22VAC40-111-520-A
Description: Based on direct observations, the Provider did not ensure cribs were provided for children from birth through 12 months of age for sleep. Evidence: The LI observed a 5-month old infant sleeping in an infant seat which was placed on the floor in the Provider's bedroom away from the other two sleeping children. A crib was available in the designated child care area for the infant.

Plan of Correction: The Provider will place infants on their backs for sleep in their designated crib.
