22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was conducted today from 10:00am-11:15am. There were 4 children (9 points) directly supervised by 1 caregiver. The home, 2 staff records, 3 children?s records, evacuation drills, and policies were inspected. There were no medications on site today. Children were observed playing freely, reading books, and reviewing colors and shapes. There was a supply of books, toys, and materials for the children. The home was organized. Standard #: 22VAC40-111-780-B Description: Based on three records reviewed, the provider did not review with the parent the emergency contact information to ensure the information is correct annually. Evidence: Child B and Child C's records were last reviewed in 2/2017. Plan of Correction: I will have the parents update the record tonight.
Standard #: 22VAC40-111-60-B Description: Based on three records reviewed, not all children's records were complete. Evidence: Child A did not have two emergency contacts, medical insurance, insurance policy number, doctor's name, doctor's phone number, and written authorization to provide emergency care. Plan of Correction: Today when parent comes I will have her fill out the information before she leaves.
Standard #: 22VAC40-111-90-A Description: Based on three records reviewed, the provider did not obtain documentation of immunizations before a child may attend the family day home. Evidence: Child A does not have documentation of immunizations on file. Plan of Correction: Parents will take child to doctor and get the information.
