Technical Assistance:
Visiting children, including family members, count in capacity and points if they are in care in the child care program.

An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 9 children, totaling 22 points. Children's records, staff records, background checks, children's nutrition, emergency preparedness items, and the physical plant were inspected today. The children were observed playing with toys, building with a train set, assembling puzzles, and preparing for lunch. The children were to be served fish sticks, whole wheat bread, oranges, hashbrowns, and milk for lunch today. This inspection was completed between approximately 10:15am and 11:50am. Standard #: 22VAC40-111-100-A
Description: Based on a review of children's health records, it was determined that one record does not include a physical exam report. Evidence: There is no physical exam report on file for child D, who began care in July, 2017.

Plan of Correction: The parent will be reminded to bring his physical.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #1 does not include the father's work address.

Plan of Correction: The parent will be asked for the information.

Standard #: 22VAC40-111-90-C
Description: Based on a review of children's health records, it was determined that one child does not have a current shot record. Evidence: Child D is age 11 months. His last shot record was dated at the age of 2 months. There is no documentation showing that this child has an updated shot record every 6 months.

Plan of Correction: I will ask the parent for a updated shot record.

Standard #: 22VAC40-111-580-B
Description: Based on staff interviews, it was determined that the provider has not provided outdoor play activities daily throughout the winter. Evidence: It was reported to the Inspector that the children have rarely gone outside for vigorous play throughout the winter.

Plan of Correction: We have play activities set up in the garage. We've been using that area during the winter. I will make sure we walk outside and/or walk to the playground daily if the weather permits.
