Comments:
An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 9 children, totaling 20 points. The children were observed assembling puzzles, playing with toys, finishing lunch, and preparing for their afternoon nap. Children's records, adult caregiver records, background checks, the physical plant, emergency preparedness items, and children's medications were inspected today. The children were served beans with rice, broccoli, and milk for lunch today. This inspection was completed between approximately 12:00pm and 1:20pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of children's health records, it was determined that one record was not complete. Evidence: There is no physical exam report on file for child #3. This child entered care in 6/17.

Plan of Correction: I emailed the parent during the inspection.

Standard #: 22VAC40-111-50-A
Description: Based on observations, it was determined that the provider has not completed all required administrative documents. Evidence: The provider did not maintain a written record of attendance for April 2 and today April 3, 2018.

Plan of Correction: I will remember to do my attendance each morning.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that four records were not complete. Evidence: 1. Child record #1 does not include: the information for a second emergency contact; documentation of an annual review by the parent. 2. Child record #3 does not include: a phone number for each emergency contact person; the parent's medical insurance policy number. 3. Child record #4 does not include the information for a second emergency contact. 4. Child record #5 does not include an annual review by the parent.

Plan of Correction: Each parent will be asked for the missing information.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's health records, it was determined that one record is not complete. Evidence: There is no shot record on file for child #3.

Plan of Correction: I emailed the parent during the inspection.
