Technical Assistance:
There are hazardous landscaping items on the ground in the backyard. The children shall not be permitted to use the backyard play area until these items are removed. The provider shall provide safe outdoor play time in a different area until the backyard hazards are removed.

An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 10 children, totaling 24 points. Children's records, children's medications, adult household records, staff training records, background checks, the physical plant, children's nutrition, and emergency preparedness items were inspected today. The children were observed creating spring art projects, building with blocks, playing with creative toys, and interacting with each caregiver. The children were to be served grilled cheese sandwiches, lentil soup, fruit salad with sliced grapes, and milk for lunch later today. Standard #: 22VAC40-111-710
Description: Based on a review of children's medications, it was determined that one prescription medication does not include all required labeling and packaging. Evidence: One child's prescription medication was on site. This medication does not include the original prescription label and original packaging.

Plan of Correction: I will ask the parent to bring the medication box with the label attached.

Standard #: 22VAC40-111-110-A
Description: Based on a review of children's health records, it was determined that one shot record was not accurately completed. Evidence: The shot record for child #3 does not include the physician's signature on the statement indicating that the child is adequately immunized for her age.

Plan of Correction: I will ask the parent to have the statement signed by the doctor.
