Comments:
A Subsidy Health and Safety inspection was conducted on April 5, 2018 to review supplemental health and safety requirements. Two caregivers were present with eight children, two of which are not enrolled. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDHOM-000-012-F
Description: Based on review, vendor did not ensure 1 of 2 caregivers is currently CPR and first aid trained. Evidence: On April 5, 2018, caregiver #2 does not have current CPR and first aid certification as it expired February 4, 2018.

Plan of Correction: The vendor will email to get it scheduled.

Standard #: VENDHOM-000-029-B-3
Description: Based on review, vendor did not ensure required procedures are documented for evacuation and relocation. Evidence: On April 5, 2018, the emergency preparedness plan does not address accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during evacuation or relocation.

Plan of Correction: The entire emergency preparedness plan will be updated.

Description: Based on review, vendor did not ensure required procedures are documented for shelter-in-place. Evidence: On April 5, 2018, the emergency preparedness plan does not address accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during shelter-in-place.

Plan of Correction: The entire emergency preparedness plan will be updated.

Description: Based on review, vendor did not ensure required procedures are documented in the emergency preparedness plan. Evidence: On April 5, 2018, the emergency preparedness plan does not address lockdown procedures.

Plan of Correction: The entire emergency preparedness plan will be updated.

Standard #: VENDHOM-000-030-A-3
Description: Based on review, vendor did not ensure lockdown procedures are practiced at least annually. Evidence: On April 5, 2018, there was not documentation of a lockdown drill conducted in 2017.

Plan of Correction: Once the procedures are written, a drill will be done.
