Today an unannounced renewal inspection was conducted with the Provider. This visit took place in the morning between 9:30am-12:45 there were 9 children with Provider and 1 assistants for a total of 30 points within the supervision guidelines. The children were observed having free play, playing with blocks, gross motor activities, sliding down the slide, playing with balls, lunch and nap time. Positive interactions were observed. A complete inspection of the physical plant, children and provider records, fire drill log, and emergency procedures were observed. Standard #: 22VAC40-111-960-E Description: Based on observation of the infant bottles, bottles did not include child's full name and date. Evidence: Upon arrival, 2 bottles did not include the name and or date. Plan of Correction: Provider stated that she will remind parents to label and date. Provider stated that parents change bottles sometimes and may forget to label and date.
Standard #: 22VAC40-111-60-B Description: Based upon a review of files, children's files are incomplete. Evidence: *1 of 9 files reviewed was missing parents complete employer address and phone number. (Child 1) *2 of 9 files reviewed was missing one of two designated emergency contact person (Child 2) Plan of Correction: Provider stated that she will ask parent for information.
Standard #: 22VAC40-111-320-B Description: Based on observation, outlets were missing protective covers. Evidence: Two electricals outlets in kitchen were missing protective covers. Plan of Correction: PROVIDER COVERED.
Standard #: 22VAC40-111-690-D Description: Based on observation, the diapering surface was found nonabsorbent. Evidence: The diaper changing pad was ripped/torn and no longer non absorbent. Plan of Correction: Provider removed and stated that she will purchase a new one.
